The Awards Ceremony will be held at this year’s AGM on Friday 13th July 2018 at Plungington Community Centre, Brook Street, Preston PR1 7NB
People who use our services or receive support are invited to submit nomination forms for individuals or teams in any one or more of the following categories:
Children’s Services Support Award and/or Community Services Support Award
This category invites people who use our services to nominate an employee who provides them with a professional and supportive service.
Those people who make a nomination might be, for example, a parent, carer, young person, foster carer, adopter, someone who uses one of our community projects, or a person with a learning disability.
We would particularly like to hear about employees who:
- go the extra mile to provide you with a good service
- treat you with dignity and respect at all times
- support you to have choice and involve you in decisions
- listen and respond sensitively to your individual needs
- help you to achieve your aims in life

Nominations can be emailed to nominations@caritascare.org.uk or placed in a sealed envelope and returned FAO Amanda Forshaw to 218 Tulketh Road, Preston PR2 1ES. Please write ‘Staff Nominations 2017/18’ in the bottom left hand corner of the envelope.
The deadline for submission of nominations is 5:00pm on Monday 18 June 2018
